Our trained Carers can support your care needs, and develop a home hospice or palliative care plan suited to your individual needs and preferences. This often includes:
- Personal care: Assistance with dressing, toileting, and bathing.
- Emotional support and companionship: From shopping trips to puzzles at home, our carers are eager to spend time with you.
- Housekeeping: Helping to keep your home clean and tidy.
- Linen changes: Changing bed sheets whenever you need.
- Respite for family carers: Providing short breaks for your family members, ensuring your relationship with them remains positive.
- Social interaction: Whether you enjoy discussing current affairs or prefer quiet time with a book, your carers will offer the kind of social interaction you prefer.
- Comprehensive palliative care information and support: We believe that understanding your options helps you make informed choices about your palliative care journey.
Our goal is to ensure you receive compassionate, personalised care that respects your wishes and enhances your quality of life at home.
A smooth transition from hospital to home to help you on your road to recovery.
To aid in your recovery and help you regain physical independence following a hospital stay, Home Support Services offers short-term and ongoing transition care services to support your complete recovery.
Leaving the hospital and returning home is comforting, but it is also a critical juncture in your recovery process. During this period, you may struggle with simple daily activities or encounter unforeseen challenges at home.
Potential setbacks, such as medication errors, missed follow-up appointments, wound care issues, poor nutrition, dehydration, or lack of safety measures at home, can lead to hospital readmission.
Our carers provide several services to ensure a safe transition from hospital to home and support you on your path to full recovery:
- Hospital discharge and coordination: Our carers collaborate with you and your hospital discharge planning staff to create a detailed care plan. This plan will guide your family and/or our carers to ensure a smooth and successful recovery.
- Medication supervision: Our carers help you stay organised with your medication schedule, ensuring proper administration and monitoring for any adverse effects or complications.
- Light housekeeping: Tasks like changing bed sheets or doing laundry can be physically challenging when you are first discharged from the hospital. Our carers ensure your bed is made, and you have clean sheets, towels, and a tidy home while you focus on your recovery.
Our goal is to provide the support you need to fully recover and regain your independence in the comfort of your own home.

They will visit at times you choose, spending quality time helping around the house, and providing a friendly face to enjoy at home. Our carers are available seven days a week, providing respite services on a regular, scheduled basis, for planned special events (such as travel or being out of town), or on short notice when you need help.
